If you use, recommend, distribute or sell OneTouch® Ultra® Test Strips, we want to make you aware that LifeScan is voluntarily removing and offering replacement for the following single lot of 25-count OneTouch Ultra Blood Glucose Test Strips sold in India.

Note: This is the only lot of OneTouch Ultra Test Strips covered by this notice. No other OneTouch® Products are included in this replacement program.

LifeScan is replacing this lot of OneTouch Ultra Test Strips because an internal investigation of recent customer complaints shows that some test strips in this particular lot do not meet our performance standards. Use of these test strips could result in inaccurately high blood glucose test results that may lead to improper treatment and possible injury. As a precaution, we have decided to remove and replace this lot in India to help ensure patient safety.
Please compare the lot numbers of any OneTouch Ultra Test Strips in your possession. The lot number appears on the outside of the test strip box and on the label of the test strip vial. If you do not have OneTouch Ultra Test Strip Lot 2679712, your test strips are not subject to this notice and you should continue testing per your normal routine.
How To Return Affected Product and Receive Replacement Test Strips
If you have any OneTouch Ultra Test Strips from Lot # 2679712, you should discontinue their use immediately and call LifeScan Customer Service to report how many vials of test strips you have that need replacement.
Also, please inform the LifeScan representative if you have other test strips available to test with while you wait for replacement strips. The LifeScan representative will then arrange to have replacement test strips sent to you at no charge, and will also provide information on how to return your test strips from Lot # 2679712.
