The OneTouch® Horizon™ Test Strips are for use with the OneTouch® Horizon™ Meter for quantitatively measuring glucose in whole blood.

• The OneTouch Horizon Meter reads the glucose level of a whole blood sample on a OneTouch Horizon Test Strip. The OneTouch Horizon Meter provides plasma-calibrated results to allow easy comparison with laboratory methods.
• Read package insert before using this product.
• For testing outside the body (in vitro diagnostic use).

Storage and Handling:

• Store the test strip package in a cool, dry place between 4°C and 30°C (39 - 86°F). Keep away from direct sunlight and heat. Do not refrigerate.
• Your test strip containers are designed to protect your test strips from moisture during normal storage and handling (as stated in your labeling).
• Always keep your test strips in their original container and quickly close the vial after removing a test strip. Failure to do so in a humid environment may cause spoilage and result in inaccurate readings.
• Do not transfer your test strips to a new vial or any other container. The OneTouch Horizon Test Strip vial contains a drying agent.
• Do not use your OneTouch Horizon Test Strips if your vial is open or damaged in any way as this could lead to error messages or blood glucose results that are higher than expected. Call your LifeScan Customer Service Representative immediately.
• Use each test strip immediately after removing it from the vial.
• Write the discard date on the vial label when you first open it. Discard any remaining test strips 3 months after first opening the vial.
• Avoid getting dirt or food on the test strip.
• Do not use test strips beyond the expiration date printed on the package.
• Do not bend, cut, or alter a test strip in any way.
